As a care management unit, UCC assists in coordinating comprehensive long-term care services to frail elders and adults with physical, developmental and intellectual disabilities who are Medicaid eligible and enrolled through the Family Care program.
The program is designed to maintain an individual's present level of functioning as long as possible while preventing premature or inappropriate institutionalization.
Some of the services available through this program include:
- Customized care plan and care coordination
- Ongoing monitoring and evaluation of care
- Adult day care
- Counseling
- Personal care services
- Home health services
- Chore services
- Transportation
- Assistive devices
- Durable medical equipment
- Prevention and wellness education
